Want to Sign Up for our Program?

You have the option to voluntarily align with your doctor by completing our Voluntary Alignment form. Completing a Voluntary Alignment form ensures that the doctor you currently use for routine care can better coordinate your care as part of this special program. You are not required to complete a Voluntary Alignment form to participate in the program and you may align to a different doctor at any time.

Here’s how you can sign up to participate in our special program:

You can download a Voluntary Alignment form and mail it to us or drop it off at your doctor’s office
